Case Request Form

It would be helpful and in the interests of the applicant if they follow the instructions carefully. Please print or type all responses. In answering questions, supporting documentation, labeled as “Exhibit A, Exhibit B” etc. will be helpful provided it is limited to the subject matter of the question. Supporting documents may be uploaded as .pdf or .doc files.

If you choose to use postal mail to send this request and documentation to us, do not send original documents, but rather copies. For example, if a supporting document is a page from an appellate brief, then only a copy of that page from the brief should be sent rather than the whole brief. Send Copies Only! Other than what is absolutely necessary to illustrate an answer, do not send any legal materials. The Foundation is unable to return any documents or materials. Supporting documents may be uploaded as .pdf or .doc files.

The Foundation accepts non-DNA cases in NY, Connecticut, and New Jersey and DNA cases nationwide.
